Why USPS Form 1583 Is Required?


Any person or business that rents a mailbox directly from an approved third party (CMRA) must complete form 1583. When submitted to the United States Post Office, form 1583 will be stored and kept on file to identify and track addressees who use this service for any unapproved or illegal purpose.

Allowed identifications to get it notarized:
Two forms of identification are required for the completion of this form. Credit cards, social security, and birth certificates are not accepted. In addition, one form of identification must be a photo ID. Here is a list of what they will accept from you:

  • Alien registration card or certificate of naturalization
  • Current lease, mortgage, or Deed of Trust
  • Voter or vehicle registration card
  • Home or vehicle insurance policy
  • Valid driver's license or state non-driver's identification card
  • Armed forces, government, university, or recognized corporate identification card
  • Passport

Step by Step Instructions:


1. Private Mailbox (PMB) Information.

  • 1a: Date you signed up to the Virtual Office in Month D, Yr format.
  • 1b: Leave blank.


2. Commercial Mail Receiving Agency (CMRA) Place of Business Information

  • Provide the address and the Private Mailbox number of the Virtual Office you signed up for.


3. Type of Service Requested

  • If the Virtual Office address is for business use, tick the ‘Business/Organization Use’ box. You will also need to fill out Section 7.


4. Name of Applicant 

  • Please provide your full name, home address, phone number, and email address. Please note that your address must match the address ID provided in Section 9b. If you are under court protection, indicate "Yes" in Section 4k and attach a copy of your court order.


5. Authorized Individual

  • If you wish to designate someone else to receive your mail as an Authorized Individual, please provide their information in Section 5. This includes their name, address, phone number, and email address. Please ensure that their address matches the address ID in Section 11b.


6. If Transferring PMB Mail to Another Address

  • Please provide your forwarding address and contact information here if you plan to have your mail forwarded.


7. Business/Organization Information

  • If you plan to utilize your Virtual Office address for business purposes, as outlined in Section 3, please provide your business information in Section 7. This should include your business name, type, address, phone number, and registration location. Note that the address entered here must differ from your Virtual Office address. For domestic businesses, provide the county and state, and for foreign businesses, indicate the country of registration.


8. Photo ID Information for Applicant

  • Enter your information per your photo identification. Specify the type of ID in Section 8e. Keep in mind that you need two forms of ID, with your photo ID and address ID being distinct.


9. Address ID Information for Applicant

  • Please provide your information based on your address ID. Specify the type of ID in Section 9g and remember to include copies of both your photo and address ID documents.


10. Photo ID Information for Authorized Individual (if applicable)

  • If you've designated Authorized Individual to collect mail for you, input their information based on their photo ID. Remember, both their photo ID and address ID should be different and included as two separate forms of identification.


11. Address ID Information for Authorized Individual (if applicable)

  • If you've appointed an Authorized Individual to collect mail for you, provide their information based on their address ID. Remember to attach copies of their photo ID and address ID documents. Your authorized individual must also attend the notary session and verify their identity alongside you. If you don't plan to nominate an authorized individual, leave sections 10 and 11 empty.


12. Exceptions for Additional Recipients of Mail

You may include additional individuals to receive mail at this address:

  • For Business/Organization Use: Specify members authorized to receive mail at this PMB. Each person listed must be able to provide two valid forms of ID upon request by the Postal Service.
  • For Residential/Individual Use: A parent or guardian can collect mail on behalf of a minor by listing the minor's name — the minor's ID is not mandatory.


13. Signature of Applicant

  • Please sign and date this section of the form during your notary session.


14. Signature of Witness

  • The notary will sign and date this section of the form during your notary session.
