Why is USPS Form 1583 required?

Any person or business renting a mailbox directly from an approved third party (CMRA) must complete the USPS Form 1583 to authorize the CMRA to handle their mail. When submitted to the United States Post Office, Form 1583 will be stored and kept on file to identify and track addressees using this service for any unapproved or illegal purpose.

Allowed identifications to get it notarized:

Two forms of identification are required to complete this form: a photo ID to verify your identity and basic information, and a different one to verify your address. Credit cards, social security, and birth certificates are not accepted. Here is the full list of what you can use:


Allowed photo ID types if you are a US citizen/resident with a US-issued SSN:

  • U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card*
  • Uniformed Service ID
  • Passport
  • Certificate of Naturalization
  • U.S. Access Card
  • Matricula Consular
  • U.S. Permanent Resident Card
  • U.S. University ID Card
  • NEXUS Card


Allowed photo ID types if you are an international customer who is not a US citizen/resident and does not have a US-issued SSN:

  • Passport


Allowed address ID types:

  • U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card*
  • Current Lease
  • Home or Vehicle Insurance Policy
  • Mortgage or Deed of Trust
  • Vehicle Registration Card
  • Voter Card


*Note: While the U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card is listed as an option for both the photo and address IDs, it may only be used for one of the IDs, not both.

Otherwise, if you would like to fill out and finalize the form some other way, manually, here are the steps you can follow:

  • Section 1. Private Mailbox (PMB) Information

    • 1a. Enter the date when you signed up for the service and were assigned a PMB. 
      Date format should be (Month DD, Year); for example, June 10, 2025 
    • 1b. This section can be left blank; your operator will enter the date if/when you cancel your subscription with them.
  • Section 2. Commercial Mail Receiving Agency (CMRA) Place of Business Information 

    Enter the CMRA address and PMB# assigned to you, which is the exact mailbox address and number in the welcome email you received when you signed up:
    • 2a. CMRA's street address
    • 2b. Your assigned PMB#
    • 2c. CMRA's city
    • 2d. CMRA's state
    • 2e. CMRA's ZIP code

  • Section 3. Type of Service Requested

    Check the correct box for the type of service based on whether you will use the PMB address for personal or business purposes.
    Note: If the "Business/Organization Use" box is checked, Section 7 must be filled.


  • Section 4. Name of Applicant

    Enter your name and personal information.
    Note: Address information must match the document provided in Section 9.
    • 4a. Your last name
    • 4b. Your first name
    • 4c. Your middle initial (if applicable)
    • 4d. Your phone number
    • 4e. Your email address
    • 4f. Your street home address (first line of the address, including house number, street, and apartment/suite number, if applicable) 
    • 4g. City
    • 4h. State 
    • 4i. Zip code
    • 4j. Country
    • 4k. Check "Yes" if you are a court-ordered protected individual with an existing order of protection or restraining order; otherwise, check "No".
      Note: If you check Yes, a copy of the court order must be attached to the form.

  • Section 5. Authorized Individual 

    Fill out if you want to authorize another individual/agent to collect mail on your behalf.
    • 5a. Authorized individual's last name
    • 5b. Authorized individual's first name
    • 5c. Authorized individual's middle initial (if applicable)
    • 5d. Authorized individual's phone number
    • 5e. Authorized individual's email address
    • 5f. Authorized individual's street home address (first line of the address, including house number, street, and apartment/suite number, if applicable)
    • 5g. City
    • 5h. State
    • 5i. Zip Code
    • 5j. Country

  • Section 6. If Transferring PMB Mail to Another Address

    Enter your forwarding address and the rest of the same information as in Section 4
    This section must be filled out if the mail addressed to this PMB will be transferred, mailed, shipped, or emailed to another address.


  • Section 7. Business/Organization Information

    If the box for "Business/Organization Use" is checked in Section 3, this section must be filled.
    Enter your business address and information:

    • 7a. Your business name
    • 7b. Your business type
    • 7c. Your business address (first line of the address, including house number, street, and apartment/suite number, if applicable)
    • 7d. City
    • 7e. State
    • 7f. Zip code
    • 7g. Country
    • 7h. Your business phone number
    • 7i. Place of registration for your business (county and state if domestic, country if foreign)

  • Section 8. Photo ID Information for Applicant

    • You must choose the document you want to use to verify your identity from the list of allowed Photo ID types, then fill out the ID information in this section.
      Note: While the U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card is listed as an option for both the photo and address IDs, it may only be used for one of the IDs, not both.
      • 8a. Your name (as stated on the ID)
      • 8b. Your ID Number (as stated on the ID)
      • 8c. ID's issuing entity
      • 8d. ID's expiration date
      • 8e. Photo ID type (check the box for the one you will be presenting)

  • Section 9. Address ID Information for Applicant

    • You must choose the document you want to use to verify your home address from the list of allowed Address ID types, then fill out the ID information in this section.
      Note: While the U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card is listed as an option for both the photo and address IDs, it may only be used for one of the IDs, not both.
      • 9a. Your name (as stated on the ID)
      • 9b. Your street home address (first line of the address, as stated on the ID, including house number, street, and apartment/suite number, if applicable)
      • 9c. City
      • 9d. State

      • 9g. Address ID type (check the box for the one the customer is presenting)

  • Section 10. Photo ID Information for Authorized Individual (if applicable)

    • If Section 5 was filled out, and you would like to add an authorized individual to collect mail from the mailbox location on your behalf, the authorized individual's photo ID information needs to be filled out in this section, and the individual needs to be present with you during the notary session to verify their identity.
      Note: While the U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card is listed as an option for both the photo and address IDs, it may only be used for one of the IDs, not both.
      • 10a. Authorized individual's name (as stated on the ID)
      • 10b. Authorized individual's ID number (as stated on the ID)
      • 10c. ID's issuing entity
      • 10d. ID's expiration date
      • 10e. Photo ID Type (check the box for the one the authorized individual is presenting)

  • Section 11. Address ID Information for Authorized Individual (if applicable) 

    • If Section 5 was filled out, and you would like to add an authorized individual to collect mail from the mailbox location on your behalf, the authorized individual's address ID information needs to be filled out in this section, and the individual needs to be present with you during the notary session to verify their information.

      • 11a. Authorized individual's name (as stated on the ID)

  • Section 12. Exceptions for Additional Recipients of Mail

    • If you need to add additional recipients to use the same PMB# to receive mail items under their name, you will need to list them here.
      • For Business/Organization Use: 
        • List the members who will be receiving mail at this PMB. 
        • Each person listed must, upon request, present two forms of valid ID to the Postal Service.
      • For Residential/Individual Use: 
        • A parent or guardian may receive the mail of a minor by listing the minor’s name 
        • The minor’s ID is not required.

  • Section 13. Signature of Applicant

    • 13a. Your signature (signed during the notary session or in front of the agent or authorized employee)
    • 13b. Date this was signed 
  • Section 14. Signature of CMRA or Authorized Employee

    • 14a. Signature of the agent, authorized employee, or notary personnel (signed during the notary session)
    • 14b. Date this was signed.
